INSURANCE & PAYMENT OPTIONS:
Our goal is to help minimize your out of pocket cost while maximizing your insurance benefits. As a courtesy to our patients, we are happy to file a pre-treatment estimate with your insurance company prior to any needed treatment. We are also happy to provide you with a written estimate, however, please be aware that this is not intended to be a guarantee benefits, as unexpected changes can occur during treatment and your insurance company estimate can change.
INSURANCE PLANS:
We accept and work with ALL major dental insurance plans! Some “PPO” plans provide a lower benefit for “out of network” providers, but there are usually benefits available to our patients.
In addition, we are a participating provider for:
- Delta Dental – Premier Provider
- Humana – Preferred Provider
- WEA Insurance Trust – Preferred Provider
For your convenience, we file the required forms to help you gain the full benefit for the services we provide. Because insurance is an agreement between you and your insurance company, all patients are responsible for all non-covered charges incurred.
PAYMENT OPTIONS:
We are pleased to offer a 5% courtesy discount for payment in full on the same day of service when payment is made by cash or check.
For your convenience, we also accept Visa, MasterCard, or Discover.
Monthly Payments can be arranged.
- Automatic credit card deductions.
- With established good credit, we can bill you. Payment in full is expected within 21 days of statement date.
- Other payment options are available such as the “Citi HealthCard”, which can help spread payment out for up to 4 years – Please call to discuss this option.
